Erectile tissue needs regular blood flow to stay healthy. When that blood flow stops — after prostate surgery, during radiation or hormone therapy, or as circulation declines with age and conditions like diabetes — the tissue begins to change. Penile rehabilitation is the practice of restoring that blood flow deliberately, on a schedule, during the period when the body can't produce it on its own.
Erections aren't only for sex
Most men don't know this: a healthy man has several erections every night during sleep, without being aware of any of them.
That nightly cycle isn't about desire. It's maintenance. It's how penile tissue receives oxygen, gets stretched, and keeps its smooth muscle conditioned — the same way any tissue in the body stays healthy through regular use and circulation.
Prostate surgery disrupts the nerves and blood vessels that produce those erections. Radiation damages them gradually. Hormone therapy removes the testosterone that drives them. In each case, the nightly maintenance cycle stops — and the tissue is left without its oxygen supply for months or years.
What happens to tissue without blood flow
Researchers have studied this closely. A 2013 review in Translational Andrology and Urology found that vacuum therapy after prostate surgery works by increasing arterial blood flow while counteracting three specific processes that damage erectile tissue during recovery: cell death, scarring, and oxygen deprivation.
In practical terms, tissue deprived of oxygen becomes more fibrous and less elastic. Elasticity is where natural length comes from.
